Overview

Dear Client, thank you for making an appointment at Bodhi Medical Care, LLC. It is our hope that you will find our service satisfactory to your needs.

Though this is only a brief introduction, we hope we can answer at least some of the most frequent questions our clients ask us prior to arrival for their appointment.

Our Medical Providers

The adult medical services are provided by board certified internists and family physicians as well as by subspecialists in various fields of Internal Medicine. Women's health services including gynecology are provided by gynecologists and family physicians

BODHI MEDICAL- Columbus circle

330 West 58th Street, Suite 414 (same building 58th street side between 8th and 9th Avenues)

315 West 57th Street, Suite 404 (same building 57th street side between 8th and 9th Avenues)

We are located directly across from the entrance to the Time Warner Building.
Location

Subway: 1/2 block from subway lines A, B, C, D, 1, 2, 3, 9 (59th Street Station)

3 blocks from subway lines N, R, E (57th Avenue station, 7th Ave station)

Parking: Prolonged street parking is very difficult on the 58th or 57th streets. Occasionally, one hour parking can be found on 57th street, 9th avenue or 10th avenue.

There are many parking garages in the area of Columbus Circle. The most convenient parking garage is at The Time Warner Center on 58th Street, between Broadway and Amsterdam, though there are many others in the area.

Lobby

You can enter the building from either 57th or 58th Street.

From 58th Street: the entrance is 330 West 58th Street. This is the more convenient entrance, as the elevators to our office are right next to the concierge desk.

From 57th Street: the entrance is 315 West 57th Street. If you enter from 57th Street, just walk across the building's lobby to the 58th street side, and use the elevators which are located next to the 58th Street concierge desk.

At either side, you may be greeted by a concierge person and you may be asked to sign in. When you are asked where you are going, tell them Bodhi Medical Care, LLC- Suite 414.

We are located on the 4th floor on the 58th Side of the building. There are two elevators which can take you to the 4th floor. Always press the lower button, calling for elevators L-6.

Elevator #1 and elevator #3 (located around the corner) will both take you to the fourth floor in the proximity of Suite 414.

At the front desk

Welcome to Bodhi Medical. Once you arrive, one of our front desk personnel will initiate the registration process.

General registration

You will be asked about your demographic data (name, address, contact information, SSN, DOB, Phone), fill our our medical registration questionnaire and sign our general practice agreement.

You will be asked to present your insurance ID card and the eligibility will be checked.Please note that your insurance has to be active at the time of your visit in order to consider it as method of payment for your services.

Should your or your child's insurance be inactive at the time of your visit, you will be asked to provide a payment in full for the visit as if you / your child were uninsured according to our "inactive insurance policy".


If you have an acute illness

You will be asked to fill out a one page form about your medical history. After your registration is complete you will be seen promptly by the doctor.

Should you feel that your illness needs more urgent attention than waiting until your appointment, please call us at 888-603-9338 for adults or 888-603-0993 for pediatrics. Alternatively, if you feel that your condition is an emergency, please dial 911 immediately.


If you have a chronic illness You may want to consider bringing a copy of your old records, or even request that your previous care providers fax us a copy of your medical records to (212) 624-0220.

If you are coming for a general/annual check-up

In addition to the general registration process, you will be presented with an annual health screening questionnaire to evaluate your vaccine history, your lifestyle and a few other topics.

At the end of your physical, we routinely preform blood screening (i.e. common tests such as cholesterol, complete blood counts, liver and kidney function tests,etc.) and urinalysis. Prior to your visit you may want to consider whether you want the routine screening to be preformed. You may also wish to consider any additional testing such as the sexually transmitted disease surveillance ( including Chlamydia, Gonorrhea, HIV, Hepatitis, Herpes among others), thyroid tests or any other relevant testing.


Insurance matters

Insurance coverage, copays, deductibiles, payments and other financial matters

Your insurance coverage will be verified with your insurance carrier by our staff, either prior to your visit and/or during the visit.

You will be asked to present your insurance ID card and the eligibility will be checked.Please note that your insurance has to be active at the time of your visit in order to consider it as method of payment for your / your child's services.

Should your or your child's insurance be inactive at the time of your visit, you will be asked to provide a payment in full for the visit as if you / your child were uninsured according to our "inactive insurance policy".


Accepted Insurances At present Bodhi Medical Care, LLC accepts the following health plans: Aetna, Amerigroup, Atlantis, Blue Cross-Blue Shield, Care Plus, Cigna, Fidelis, GHI, Great Westcare, Healthnet, HIP, Humana, Magnacare, Medicaid, Medicare, Multiplan, Oxford, PHCS, POMCO, PPN, United Healthcare, Unicare, Vytra/HIP, 1199

Co-pay

You will be asked to pay the co-payment as assigned by your insurer. If the amount of the co-pay for an office visit is not on your card, you can call your insurer to verify the amount of your copay.


Deductible

At the time you sign your insurance contract, you are often times agreeing to a deductible. The deductible is the amount you have agreed to pay out of pocket before your insurance starts paying for anything regarding your healthcare.

Some patients have $0 deductible, some have a $500 and some $2500 deductible, or more. The amounts vary. However, if you have not met your deductible through your insurance, you will be asked to pay for your office visit in full at the time of check out.

Thus, it is important that you call your insurer and verify your deductible.


Extra services

Some insurances do not routinely cover "annual physicals". GHI insurer is one common example. Many insurances do not cover employment related physicals, tuberculin skin tests, tetanus vaccines boosters or travel immunizations. If you have such requests that are not covered by your insurer, or are unsure whether certain visits or tests will be covered, please contact your insurer.

Our office also provides some services generally not covered by the insurers such as filling out school, work or camp forms. Please review our "form fees" as well as other general fees for uninsured.


Uninsured

We try to accomodate those without insurance. Our usual charge of an office visit is $100.if applicable, please review our general fees.

Additional in office or send out laboratory testing, if required, is not part of the standard fee. Further examples of testing not covered by the flat fee are EKG's, spirometry, travel vaccines etc.


Payment methods

We accept cash, checks, as well as credit cards (through our online- Bodhi Medical Care, LLC -paypal system).

The parents/ responsible parties who are NOT present at the time of the visit and wish to extend a credit card payment for the service of their family member MUST sign an agreement for the payment.


Blood work
We routinely draw blood for laboratory testing in our office. We use national laboratory systems such as Quest diagnostics and Enzo Laboratories for most of our patients. Charges for any blood tests are handled directly between the laboratory and your insurer. Our patients infrequently experience any problems with the insurance coverage of their laboratory tests. Most of the time, no prior approvals are necessary.

 


Imaging

We use several specialized diagnostic imaging facilities in the area, depending on the procedure, insurance and particular patient needs.

The most frequently used imaging centers include the following: Park West Radiology, West Side Radiology, Doshi Diagnostics, MRI on 11th, and a few others. We will be more than glad to refer you to a center that is most convenient for you.
